Reducing Readmissions and Length of Stay
The major reasons cited for patient readmission following a hospitalization are severe progressive illness, failures in discharge planning, and insufficient outpatient and community care. The goal of this webinar is to identify end-stage chronically ill patients that may benefit from palliative care / hospice care in their care continuum and provide comprehensive care to these patients, thereby reducing hospital readmissions. We will accomplish this by identifying the role of chronic, terminal illness in hospital readmissions, discussing strategies used by hospice providers to prevent readmissions, discussing barriers to hospice election and discussing the benefits of hospice care for patients with terminal illness.

As readmissions have proven to be both a quality and financial burden on the health care delivery system, the Hospital Readmission Reduction Program (HRRP) was established as part of the Affordable Care Act in order to achieve meaningful reductions in all-cause readmissions by aligning reimbursement to the outcome of reduced readmissions. One way to assist providers in reducing readmissions is by partnering with hospice and palliative care programs. By providing comprehensive services to patients wherever they live, be it in a private residence or a long-term care facility, hospice can keep patients from returning to the hospital. In the event that inpatient care is needed, these services are provided by the hospice as well, again avoiding readmission. For patients who are not eligible for hospice services, palliative care services, which extend hospice-like services to a broader population, also can enhance discharge planning and care coordination. Both hospice and palliative care services have been shown to reduce hospital stays and readmissions as well as cost of care, while improving quality patient care and patient/family satisfaction.
